Remove an add-on from a tenant plan

Locked transaction: `qty -= n`, `limits[limitKey] -= delta × n` (never below 0), the entry removed at 0, `addon.removed`. Usage above the new limit deletes nothing. 409 `Only <n> added.`; 404 when the plan holds no such add-on.
