Apply a tier to a tenant plan

Moves the tenant plan onto the tier: `limits` = the tier CURRENT limits + every add-on the plan holds (kept even above the new maxQty). Dates and add-ons are unchanged; the same tier again = reset to tier. Writes `plan.applied`. 400 `tier: Not a tenant tier.`, 404 `tier: Plan not found.`
